Calling All Coding Geniuses

Hello everyone. Greg and I need your help.

It has come to that time of year again that we need to have someone create a "Real Life Comics Demographic Survey". So if you know how to do this and can help us out.... we need you! Here is what we need:

1) A survey that only allows you to submit ONCE.
2) One that will total up the results for us to view.
3) Can have both radio buttons and drop down questions.

I don't care if it is done in flash or not. I don't care how pretty it looks. I just need to have it so we can post a link on the front page for people to go to and submit their answers.

The person that can do this for me will get a copy of the Real Life Year One book signed by Greg. I hope that sounds like fair payment. If you don't want the book, then we can figure out some other type of item to send you.

If you think you can do this for me, then send me and e-mail and I will send you the list of questions for you to use. This is kind of on a first come- first to be done basis. Whoever can get the completed survey and results page to me first, will get the book. Thanks for your help.

A few people have been asking for it, so check it:

I've actually had it since about february, though. I'm proud of it, so that's all that matters. :) My next project is a 60-gallon tank I have that I'm going to turn into a saltwater setup. It'll be expensive and take lots of effort, but DAMN it'll be cool. :D

I should also mention, I contacted the artists of that UO painting, and they can't reprint it, so I really have no idea where to get a good-sized version of it. Perhaps if anyone from EA is reading, and they're in the UO department, they could get something in motion *wink wink*.
